Diversion Team Meeting – February 28, 2011
In attendance: Alvina Brevard, DHCD, Marcia Crutchfield, HAP, Ken Demers, New England Farm Workers Council, Tom Salter, NEFWC, Pamela Schwartz, network coordinator, Eryn Tobin, Catholic Charities
Discussed draft “Diversion Team Applicant Assessment and Outcome Report” (thank you, Alvina!) to be used by all providers as assessment tool at Liberty Street office.
Alvina: will resend to all via email assessment form with proposed revisions
Marcia: will send via email HAP’s verification check-list for possible adoption in diversion efforts
Release forms: agreed that everyone will put every other agency on their release so can exchange information easily (and track which families have received assistance)
Next meeting: Monday, March 14th, 11:30 am, HAP Housing
