ICHH Site Evaluation Visit

Our ICHH Site Evaluation visit will take place Tuesday, August 11, 2009, 9 am to 3 pm, at the Western Mass regional office of the Department of Mental Health–the Haskell Building, 1 Prince St., Northampton.  The agenda is attached.  Lunch will be provided.

The following people/organizations should be present for the site evaluation:

From 9 am to 12 pm:  Key members of Leadership Council (Invited: Evan Dobelle, Lynne Wallace, Mayor Sullivan, Charlie Knight, Yasmin Otero, Liz Sullivan, Dave Christopholis, Peg Keller, Gerry McCafferty), Convening Agency rep (Tim Brennan, Jim Mazik or Erica Johnson), Coordinator (Pamela Schwartz), data analyst (not yet hired), 1 rep from each ICHH sub-contractor (ServiceNet, HAP, BHDC, Community Action, FCRHRA, HAP, NEFWC, and VOC),  rep(s) from sub-committees–including family (Yasmin Otero) and individual (Dave Modzelewski) innovation sub-committees.

 From 1:00 pm on: Leadership Council members no longer needed.  Convening Agency, coordinator, data analyst,1 rep from each ICHH sub-contractor, rep from family and individual innovation sub-committees.
