Housing Chronically Homeless Individuals / Meetings Set
Monthly meetings have now been established in all four WM sub regions (Springfield, Holyoke/Chicopee/Westfield, Hampshire/Franklin and the Berkshires for the purpose of matching chronically homeless individuals to housing and service resources. Providers in each of the 4 sub regions have begun establishing resource data bases as well as identifying individuals for housing placement, service connection or enhanced engagement. Once the data bases have been established, providers of both housing and support services will regularly update the REACH team of resource openings. The last hour of each monthly sub regional meeting will be committed to specifically matching individuals to an available housing plus support services resource. The referring agency will begin discussion with the resource agency. If the match appears to be a good one, both agencies will identify next steps and if a placement is ultimately accomplished, the REACH team will be informed and the participant and housing and support services data bases will be updated. Monthly meetings are as follows:
Springfield 736 State St. 3rd Tuesday of each month – 2:00-3:30. Next meeting March 17th.
Holyoke/Chicopee/Westfield 53 Southampton Road, Westfield ( DMH Site Office) 3rd Wednesday of each month 1:00-2:30. Next meeting March 18th.
Hampshire /Franklin 1 Prince Street, Northampton (DMH Haskell Bldg) last Monday of each month, 1:00- 2:30. Next meeting March 30th.
Berkshires Next meeting 333 East Street, 1st floor Conf. RM, March 17, 11:00-12:30 Regular monthly date to be set.
